I don't think you can do that. You would need to pay the outstanding payments on the lease first, which is basically to turn in your current lease and get a new one. You can do that when you have 3 or less months left, and even then you will probably be charged part of the outstanding lease payments.
DANG I wouldn't mind considering taking over your lease. I am sure you avoid a lot of fees and crap (though I never done a take over) but its A/T =/ You know I bet there is a website that can help you find someone to take over your lease.
You should have just leased the car for a shorter amount of time. Sure the payment may have been higher but I would imagine they would be less than the penalty fees you will pay to get out of your current lease early.
